Information For New Clients

As a professional relationship, we need to agree on the terms of our work together. This is spelled out in the Client Information and Consent for Services document and the Georgia Notice Form. Please read these documents thoroughly. When we set up an initial appointment I will invite you to set up a client portal, where you can update all of your information, including reading and signing the consent form which represents your understanding of and agreement to these terms. Additionally, to give us the option of using telemental health, please read and sign the telemental health addendum which will also be found on the client portal.

Professional relationships are also characterized by payment or compensation for services. My fee is $175 for a standard 45-minute session. The cost to you may be offset if you will be using insurance. I accept cash, checks, and credit cards for payment. You may enter your credit/debit card information through the client portal.

A scheduled appointment means that I am holding a block of time for you. I will provide you with an automated appointment reminder 48 hours in advance. A missed or canceled appointment with less than twenty-four hours’ notice will result in a charge of $75. Please note that insurance will not cover this fee.

Prior to our first meeting, please update your profile in the client portal, read and sign the consent forms, complete the biographical information, and enter your credit/debit card information.
